Join the practice

 We welcome new patients, it is quick and easy to register with the practice. No need to attend surgery, you can complete the online registration form, or if it is more convenient, contact the surgery and one of our helpful members of our admin team, will be able to complete the paperwork for you over the telephone. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please call 01204 468161 for further information.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registrations


You can be registered as a temporary patient, if you are staying in the area between 1 night  for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to register as a  permanent patient with the practice.
